Shannon MyChart Medical Record Access Terms and Conditions

Acceptance of Terms and Conditions is required to access your Medical information online.

What is Shannon MyChart Medical Record Access?

Shannon MyChart Medical Record Access is an Internet application that enables a patient to have secure web-based access to portions of their Shannon medical record, and allows a secure, convenient communications channel with Shannon for non-urgent messages. Use of this service is optional. Your login name and password are key to protecting the confidentiality of your information. It is your responsibility to keep your password confidential. It is only known by you and can be changed online at any time by you. Your password is not known by anyone at Shannon.

Secure Messaging Guidelines
Please observe the following guidelines for messages sent through Shannon MyChart:
1. Please DO NOT SHARE your password with anyone.
2. Please do not send any messages requiring urgent attention. Typical turnaround time for responses is one (1) to two (2) business days.
3. Please note that Shannon MyChart messages may be viewed by other health professionals and others who work with your physician.

Your Responsibilities
By accessing or using Shannon MyChart you agree to be bound by all the terms and conditions of this Agreement and the Shannon MyChart Privacy Statement as posted. We may modify this agreement and the privacy statement at any time, and you agree that such modifications are effective immediately upon posting of the modified version. You agree that Shannon MyChart is a communication service offered as a convenience to Shannon patients. You understand that it provides you online access to portions of your Shannon medical record and that you are solely responsible for any sharing of its content that you intentionally or unintentionally communicate to others. You understand that messages will be routed to the appropriate department as necessary for handling, and therefore Shannon employees, other than your healthcare provider may be involved in addressing your request. Therefore use of Shannon MyChart may not be appropriate for very sensitive information you wish to discuss directly with your provider.
You understand that you may receive messages, test results and other communications delivered via Shannon MyChart. Therefore, you may not receive other types of communications from Shannon in these areas.

 

Use of Third-Party Services

You may choose to use other services provided by third parties in conjunction with your use of MyChart. Such services may include, but are not limited to, translation services, location services, and other third-party offerings. Any links to such services are provided for your convenience only. Neither Epic nor Shannon have control over the contents of these services, and neither Epic nor Shannon accept any responsibility for them or for any loss or damage that may arise from your use of them. Such services may require you to submit your information to a third party, which may result in a copy of your information be used and/or stored by a third party. If you decide to access any such services in conjunction with your use of MyChart, you do so entirely at your own risk and subject to the terms and conditions of use for such services.

Voluntary Deactivation of Shannon MyChart Medical Record Access
You can voluntarily disenroll from your use of Shannon MyChart Medical Record Access at any time by completing the appropriate online request to disenroll or by calling your physician office.

Involuntary Deactivation of Shannon MyChart Medical Record Access

Access to Shannon MyChart is a courtesy and not a patient right. Shannon reserves the right in its discretion to discontinue your access if Shannon believes you have violated these terms and conditions or abused the use of this service.  The following is a list of reasons which may result in deactivation. The list is not inclusive and each circumstance will be evaluated individually.

 

  • Automatic system deactivation will occur for a parent to view a child’s record when the child reaches the age of 17
  • Inappropriate use of Shannon MyChart (examples include: excessive, unnecessary, inappropriate content in messages or use of scheduling)
  • Abuse of proxy access
  • Pending legal issues with patient or direct family

 

You will be notified if we deactivate your Shannon MyChart medical record access.  You may always obtain a copy of your medical records by sending a written request to the medical records department.

Governance
This Agreement shall be governed by and construed in accordance with the laws of the state of Texas.

 

I understand that Shannon MyChart may not be available to me all the time due to system failures, back-up procedures, maintenance, or other causes beyond the control of the Service Provider. Access is provided on an “as-is, as-available” basis and Shannon does not guarantee that I will be able to access Shannon MyChart at any particular time.

 

I understand that Shannon takes no responsibility for and disclaims any and all liability arising from an inaccuracies or defects in software, communication lines, private networks, or Internet connectivity. This access to my medical record is provided as a service.  If I should have difficulty understanding something in my record or feel that anything that I view is inaccurate, I will contact my medical provider for further information.
